Step 1: Print out the Birth Center Profile Form and complete it.
Step 2: Print out and copy the UDS Data Collection Form to begin collecting patient data.
Step 3: Log-on to the UDS at http://uds.birthcenters.org and complete the birth center profile. You will not be able to save your form until is complete. This why you need to fill out the paper version first.
Step 4: Administrator - review and approve the Birth Center Profile. This must be done before you can begin entering patient records.
Step 5: Create Patient(s). You can create patients as you enter records.
Step 6: Enter data.
Note: Section 1 must be approved before you can enter data in Sections 2 and 3. Items marked with asterisk (in bold on the paper form) are required.
You can enter data retrospectively, however, our goal is for prospective data. All data entered prospectively will be marked as such.
Step 7: Administrator - Approve Records
The record approval process is the process to assure clean data. Please review carefully and thoroughly for accuracy and completeness. Failure to do so will result in invalid data for you and researchers at the national level. Once a record has been approved, you cannot make changes. Once a record is approved it will be used in report analysis.
Step 8: Run Reports and/or Export Data- Only approved records will be used in report analysis or exported.
